Prime Horizon supported apartments offer independence in a safe environment with the added  security of trained staff. Our apartment program enables the people we support to make choices, increase their independence, and plot their own life course.

Our services will wrap-around our clients. Prime Horizon assesses each individual, based upon their skill levels we create programs that fit the unique needs of each individual we serve. We recognize not everyone likes to live alone, therefore, if interested we will also work with each person to find them an ideal roommate. Before matching anyone with a roommate, we will arrange for multiple meet-and-greets between the perspective roommates. This will allow them to get to know each other to determine if they are interested in living together in an apartment. These meet-and-greets will be arranged and attended by our trained staff. During this process Prime Horizon will be locating potential apartments for the clients to view. Once an apartment is located, our clients will then tour them, and if interested, a lease will be signed. Each apartment has approximately 25-30 hours of staffing.

  • Linen change/laundry (oversight and supervision)
  • Money management/household bills/weekly budgeting
  • Regular doctor appointments and medical coordination
  • Supervision and assistance of household cleaning
  • Medication oversight and changes, pharmacy communication, maintaining durable medical equipment (walkers and wheelchairs)
  • Work with our clients to assist them with transportation coordination for recreational activities, to and from work, to medical and non-medical appointments,
  • 24-Hour On-Call trained staff for our clients for any questions, concerns or emergencies
  • Behavior planning and follow through
  • Communication with family, guardians and medical staff
  • Medical treatment plan set-up, follow through and implementation
  • Communication and coordination with property management company
  • Move in/move out coordination
  • Assistance in finding and maintaining community employment
